How to file a report via Time To Score


Report Writing Committee

The primary goal of this committee will be offering is post game advisement. Any member who has topics of concerns pertaining to penalty assessment, game management or simple reporting logistics, please reach out to us. This committee recognizes post game duties may be cumbersome at times presenting moments of uncertainty or limbo. We have opened our doors of communication and are willing to provide assistance in finding answers or clarification.

How to report a Game Misconduct

*You have 48 hours to report a Game Misconduct*

Proceed to the USA Hockey Reporting System and complete the following:

  1. Log in, then click on “My Profile”;
  2. Select “Officiating Information”;
  3. Select “Submit an Incident Report”;
  4. Complete your report.

Be sure to include a detailed report of the incident. If necessary, these reports may be used in a hearing for further disciplinary action.

After you have filed your USA Hockey report, you will be e-mailed a copy of that report in a .pdf file format. Save that file to your device.

Then head to Time To Score and complete the following:

  1. Log in;
  2. Select “File Incident Report” in the navigation panel on the left;
  3. Select the game by clicking “File Report” and;
  4. Fill in any fields and upload the file you saved to your device.

How to report a Match Penalty

*You have 24 hours to report a Match Penalty*

All CAHA/SCAHA and adult league match penalties must be submitted to gamereports@lahoa.com. This must be done within the first 24 hours of occurrence. Once report is received, this committee will review submittal and reach out to the officials.

Once you have heard back from the report writing committee, you pay proceed to file your report on the USA Hockey Reporting System:

  1. Log in, then click on “My Profile”;
  2. Select “Officiating Information”;
  3. Select “Submit an Incident Report”;
  4. Complete your report.

After you have filed your USA Hockey report, you will be e-mailed a copy of that report in a .pdf file format. Save that file to your device.

Then head to Time To Score and complete the following:

  1. Log in;
  2. Select “File Incident Report” in the navigation panel on the left;
  3. Select the game by clicking “File Report” and;
  4. Fill in any fields and upload the file you saved to your device.

Lastly, you need to report the incident via e-mail to the Pacific District Referee in Chief. You can do so by sending that same file that you saved to your device.
