How to file a game report
*You have 48 hours to report a Game Misconduct & 24 hours to report a Match Penalty*
Proceed to the USA Hockey Reporting System and complete the following:
- Log in, then click on “My Profile”;
- Select “Officiating Information”;
- Select “Submit an Incident Report”;
- Complete your report.
Be sure to include a detailed report of the incident. If necessary, these reports may be used in a hearing for further disciplinary action. After you have filed your USA Hockey report, you will be e-mailed a copy of that report in a .pdf file format. Save that file to your device.
Then head to Time To Score and complete the following:
- Log in;
- Select “File Incident Report” in the navigation panel on the left;
- Select the game by clicking “File Report” and;
- Fill in any fields and upload the file you saved to your device.
For Match Penalties:
You will need to report the incident via e-mail to the Pacific District Referee in Chief. You can do so by sending that same file that you saved to your device.
Spectator Ejections
The referee will file a report within 24 hours with USA Hockey. On the scoresheet, please document that a spectator ejection occurred. The spectator’s name should be documented on the scoresheet along with the team and what time during the game it occurred. Report it as “Other Incident” and use the players name as the input. In the report, use the parent’s name and denote that it was a spectator ejection. You do not need to report it via TimeToScore.
